Qualifications:

• Degree in Nursing (Bachelors or Associates is accepted)


• BLS


• Any State RN License

Experience:

• 2 years of experience in care coordination, discharge planning, or case management in a medical setting within the last 4 years.

Hours:  Monday through Thursday 8 am – 5 pm & Friday 8 am – 4:30 pm
